PFT IMPACT Co-Response Worker Centre/North Hastings – 2024 – 014

The IMPACT Co-Response Worker (ICW) provides mobile addiction and mental health (AMH) co-response, intervention, support, and services in a community setting in partnership with local police Services. This position will be embedded in area police service locations as an addiction and mental health resource. The ICW will assist with AMH calls for service when deemed safe and appropriate to do so. The ICW will act as a contact point person for the police services in which they are located and also liaise with the AMH and broader health system as needed (ex. TFT Addictions Counsellor -Youth – 2024 – 019

The Addictions Counsellor – Community Youth Substance Use Support & Treatment Program works as a member of a multi-disciplinary, multi-agency team to provide community-based support and treatment services, including withdrawal support, to individuals aged 16-24 years as part of an integrated community substance use program for youth. Duties include screening and assessment to ensure the most appropriate interventions are offered to the client including community or residential withdrawal and treatment services, negotiating treatment goals and a treatment plan, case management, personal/supportive counselling, liaising with Primary Care, community-based youth services and residential withdrawal services, and facilitating internal/external referrals.
