Community Homes for Opportunity (CHO) is a supportive housing program for adults with serious mental illness and provides housing, meals, assistance with activities of daily living and 24 hour onsite staffing. The program is intended to assist tenants by providing appropriate housing and support services to achieve and maintain stability in a home that is safe and affordable. The program takes a recovery-based approach and provides access to support services both in the CHO Home and in the community, which will aim to improve and/or stabilize peoples’ physical and mental health, foster independence, and enhance participation and integration into the community. The CHO program focuses on person-centred flexible services, reflecting the individual and changing needs of tenants. A recovery-based environment in CHO Homes includes supporting tenants to be empowered to be active participants in their own planning and ensuring the opportunity to make individual choices. The Program offers opportunities for tenants to enhance the quality of their daily living, their personal growth and development and improve life skills through participation in a variety of activities and programs.
AMHS-HPE currently oversees 2 CHO homes located in Kington- Keates Residence (5 beds) and Sipho’s Homes for Special Care (7 beds). Applications can be completed by filling out the online form below.
Please be advised that this program is currently full with minimal turnover; as such, AMHS-HPE operates a waitlist. When a vacancy occurs, the home operator will contact the referral agent for the next candidate on the list and invite them to complete a supplementary information package and arrange for a tour.
